The puk code is sim card spesific it comes with the sim card when you get it or you can get it by calling your phone service provider of the sim card to get the puk code.
But how can you call you phone service provider when your simcard is blocked?

I have the same problem! i was trieing to put a lock on my messages but i some how ended up locking myself out of my phone! i think all you need to do is ring the company your phone is with and they will find out other wise look on the packaging of the sim AND phone! i've been going out of my mind with out my phone. Hope you solve your problems everyone lol! x ;)

The PUK code MUST COME FROM THE SERVICE PROVIDER as it is SIM specific! I would ask to speak to a supervisor if after identifying yourself as the legal owner of the SIM they refuse to give it to you!
Most service provider will also give you the PUK code in your Pre-Paid Online management site when you register the SIM online.
