Answer from the Swiss Lebara: http://www.lebara.dk/en/faq.php I have lost my PIN/PUK code. Where can I find it? Please contact Customer Services on 0840 4800 00(free call number). Our agents will be able to provide the code after authenticating you. But you need to go to http://www.lebara.com/ and select your country to find Customer Service phone number for you (don't call the Swiss number).

Hi, To get PUK code please contact your service provider.they will provide you the defaultPUK so that you can open your phone. While you get the PUK code ,the serviceprovider customer care officer will askfor your personal details to checkwhether the mobile belongs to you only.If they are satisfied with your answers they will providewith the PUK code Note: if you keep entering random/unknown numbers/codes, the sim card will be lockedand will be rejected.
