How to write a memo about the policy of keeping x-rays at medical clinic?

I can walk you through the steps of creating a Memo, but are you sure it's a Memo you need to write? It sounds more like it should be a report rather than a memo.

I am just looking for the notation at the bottom of the memo that shows it was written by a secy/asst and not the person who is releasing it and asked for it to be written.
Can you help
